Immunotherapy

Immunotherapy, also known as biological therapy, is a type of cancer treatment designed to enhance the body’s natural defenses to fight cancer. It uses substances made by the body or in a laboratory to improve or restore immune system function.

How Immunotherapy Works

Immunotherapy works by helping the immune system recognize and attack cancer cells. It can:

  • Boost the immune system’s response: Enhancing the immune system’s ability to fight cancer cells more effectively.
  • Target cancer cells directly: Certain immunotherapies target specific markers on cancer cells.

Types of Immunotherapy

  1. Monoclonal Antibodies
    • Function: These are lab-produced molecules engineered to serve as substitute antibodies that can restore, enhance, or mimic the immune system’s attack on cancer cells.
    • Example: Rituximab for lymphoma, Trastuzumab for breast cancer.
  2. Immune Checkpoint Inhibitors
    • Function: These drugs help the immune system recognize and attack cancer cells by blocking the proteins that stop immune cells from attacking the cancer cells.
    • Example: Pembrolizumab (Keytruda), Nivolumab (Opdivo).
  3. Cancer Vaccines
    • Function: These vaccines work by stimulating the immune system to attack specific cancer cells.
    • Example: HPV vaccine for cervical cancer, Sipuleucel-T for prostate cancer.
  4. Adoptive Cell Transfer
    • Function: This treatment involves boosting the natural ability of your T cells to fight cancer. T cells are taken from the tumor, grown in large numbers in the lab, and then infused back into the patient.
    • Example: CAR T-cell therapy.
  5. Cytokines
    • Function: These are proteins that help stimulate and regulate the immune system.
    • Example: Interleukins, Interferons.

  • Targeted Approach: Specifically targets cancer cells, reducing damage to normal cells.
  • Potential for Long-term Remission: Some patients experience long-term remission.
  • Synergistic with Other Treatments: Can be used in combination with chemotherapy, radiation, and surgery to enhance overall treatment efficacy.

Side Effects

While immunotherapy can be highly effective, it can also cause side effects, which can range from mild to severe:

  • Common Side Effects: Fatigue, skin reactions (rash, itching), flu-like symptoms (fever, chills, weakness), and gastrointestinal issues (nausea, diarrhea).
  • Severe Side Effects: Autoimmune reactions where the immune system attacks normal organs and tissues.

Who Can Benefit from Immunotherapy?

Immunotherapy is used to treat various types of cancer, including:

  • Melanoma
  • Lung cancer
  • Kidney cancer
  • Bladder cancer
  • Head and neck cancers
  • Certain types of breast cancer
  • Lymphoma and leukemia

Not all patients are suitable for immunotherapy. Factors such as type and stage of cancer, overall health, and previous treatments are considered when determining eligibility.
